New Patient Visit
Your first visit will include a complete health history and review of your present condition. We may discuss your diet, lifestyle, stress, exercise, sleep, etc... Physical examination is conducted, your pulse taken and your tongue examined according to the Chinese Medicine diagnostic system. Then we will determine a treatment strategy that best suits your individual needs and condition. Once a thorough intake has been completed, diagnosis and treatment plan determined, any remaining time will be devoted to a treatment. Recommendations on diet, lifestyle, or therapeutic exercises may also be given. In subsequent visits, less time will be needed for intake and consultation, allowing more focus on the treatment itself. To save time on your first visit, please fill-out a New Patient Form. Also, please bring with you any relevant medical test results, such as lab tests, MRI reports, etc...

Patients Without Insurance Coverage
Full payment is due at the time of service (Cash, Check, Visa and MasterCard).

Patients With Insurance Coverage
We can verify acupuncture coverage for you. Your treating acupuncturist may be an in-network provider. However, if we are out-of-network but you have acupuncture coverage, you will be expected to pay at the time services are rendered. We will then provide you with an invoice to submit to your insurance company for direct reimbursement to you. Cash, check, Visa and MasterCard are accepted as payment options.

24-hour Cancellation Policy
If you need to change or cancel your appointment, please kindly give us at least 24 hours advanced notice. Failure to do so will result in being charged the full price of your appointment.

Workers' Compensation Claims
Treatment will be provided with a workers' compensation claim approval. If your employer or their insurance carrier denies your claim, you will be held financially responsible for all charges incurred for services rendered on your behalf. Any quotes given regarding treatment are cash rates - insurance may be billed differently.

  2. You can download a form-fillable PDF version of the New Patient Forms. This will allow you to fill in all the fields using Acrobat Reader. You can then print out the completed form and bring it to your first appointment.
